Your annual wellness exam is far more than a routine check-in. It’s your opportunity to catch potential health issues early, review your risk factors, update important screenings, and build a relationship with a provider who truly knows you. Prevention is always the best medicine.
What's Included in Your Annual Wellness Exam
Medical History Review
We update your complete health history, review any changes in symptoms, family history, medications, allergies, and lifestyle factors since your last visit.
Head-to-Toe Physical Examination
A thorough physical assessment including cardiovascular, respiratory, abdominal, neurological, musculoskeletal, and skin evaluation.
Vital Signs & Biometric Measurements
Blood pressure, heart rate, respiratory rate, height, weight, BMI, and oxygen saturation.
Lab Work
Based on your age, sex, and risk factors, we order appropriate blood work including:
- Complete Metabolic Panel (CMP)
- Complete Blood Count (CBC)
- Fasting lipid panel (cholesterol)
- Fasting blood glucose and/or Hemoglobin A1C
- Thyroid function (TSH) as indicated
- Vitamin D levels as indicated
Diabetes & Metabolic Screenings
- Blood glucose and A1C testing
- Prediabetes counseling and intervention
- Urine testing — urinalysis and urine microalbumin to check for early signs of kidney involvement
- Blood testing — comprehensive metabolic panel including kidney and liver function markers relevant to diabetic monitoring
- Diabetic eye exam referral — we place referrals to ophthalmology for your annual dilated diabetic eye exam, which screens for diabetic retinopathy, macular edema, and other diabetes-related vision complications. Early detection is critical — many patients have no symptoms until significant damage has occurred
- Referral to endocrinology as needed
Screenings & Referrals We Coordinate
Preventive screenings are one of the most powerful tools in medicine. Based on your age, sex, and personal and family health history, we discuss and coordinate referrals for:
Cancer Screenings
- Colonoscopy (colorectal cancer screening, beginning at age 45 or earlier based on risk)
- Mammogram (breast cancer screening for women, typically beginning at age 40–45)
- Pap smear and HPV testing (cervical cancer screening for women)
- Low-dose CT lung cancer screening (for high-risk individuals, typically current or former heavy smokers ages 50–80)
- Skin cancer screening and referral to dermatology as indicated
- Prostate-specific antigen (PSA) discussion for men beginning at age 50, or earlier for those at elevated risk
Cardiovascular Screenings
- Blood pressure monitoring and hypertension management
- Cholesterol and triglyceride evaluation
- EKG as clinically indicated
- Referral to cardiology for further evaluation when appropriate
Diabetes & Metabolic Screenings
- Blood glucose and A1C testing
- Prediabetes counseling and intervention
- Referral to endocrinology as needed
Bone Health
- DEXA scan referral for osteoporosis screening (women 65+, and younger women or men with risk factors)
Vision & Hearing
- Referral to ophthalmology for annual diabetic eye exams and routine vision care
- Referral to audiology for hearing concerns
Mental Health Screenings
- PHQ-9 depression screening
- GAD-7 anxiety screening
- Discussion of stress, sleep, and emotional wellbeing
- Referral to behavioral health providers as appropriate
Immunizations
We review and update your vaccination status, including:
- Annual influenza vaccine
- COVID-19 vaccine (updated formulations)
- Tdap / Td booster
- Shingles vaccine (Shingrix, recommended at age 50+)
- Pneumococcal vaccine (recommended at age 65+ or earlier for certain conditions)
- Hepatitis B series if not previously completed

At the end of your visit, you leave with a clear after-visit summary that includes your results, any referrals placed, upcoming screenings due, and a personalized plan to optimize your health over the next year. We also discuss nutrition, physical activity, sleep hygiene, and any lifestyle modifications that can meaningfully reduce your long-term health risks.
Medicare Annual Wellness Visit
For Medicare patients, we offer the Annual Wellness Visit (AWV) — a separate, no-cost-to-you benefit that focuses on preventive care planning, cognitive screening, fall risk assessment, and updating your personalized prevention plan. This is distinct from a comprehensive physical exam and is covered 100% by Medicare with no copay.
